Preface:
The first edition of this book was written 36 years ago by Sir Norman Browse to help medical students develop their bedside clinical skills by learning how to take a full history and to conduct a full clinical examination. The preface to the first edition lays down the principles of the book, namely detailed, systematic working through the standard headings, without which valuable clues to the diagnosis can be missed. Although the book was illustrated profusely with clinical photographs and diagrams, investigations and management were excluded specifically. The idea of a companion volume covering these was first suggested in 1980, but Browse’s Introduction to the Investigation and Management of Surgical Disease did not appear until 2010, with the same four editors as the present volume. Although the diseases themselves remain the same, their pattern of presentation, relative frequency and classification is changing constantly. Peptic ulceration in first world countries is now treated so swiftly and effectively that it rarely presents to surgeons, while diseases associated with obesity are far more common. Some formerly common conditions such as the complications of syphilis are now rare, while the introduction of screening has altered the profile of, for example, breast cancer. This is one reason for another edition. A second reason is demand to include the full range of surgical conditions. Although the original book was aimed at medical students, its systematic and highly detailed approach with stress on basic principles has made it attractive to trainee surgeons preparing for postgraduate examinations. The four editors have revised those chapters in their individual areas of expertise, but there are now many more authors, whose details are listed in full and against the relevant chapters in the Table of Contents. The sections on orthopaedics and trauma have been expanded massively and there are new chapters on cardiothoracic and neurological surgery. Investigation and management remain by and large excluded but are mentioned occasionally and briefly where they form part of clinical assessment, as for example with major trauma. The book now covers all areas of surgical disease except for otorhinolaryngology and ophthalmology. All of the line diagrams have been redrawn and there are some new clinical photographs of higher quality than before. The popular revision panels have been retained. It is ironic that Sir Norman’s single author purposefully didactic clinically orientated book now requires four editors and multiple authors. The reader may be reassured that the original approach has not changed. Even today most diagnoses can be made by taking a careful history and carrying out a full examination, and this book is intended to help the student to acquire these basic skills.
