The outline of Fractures presented a considerable challenge. The attractions to
accept were that the new 12th edition would appear 50 years after John
Crawford Adams published the first edition of his book and his wise
guidance was still available to us in its preparation. Science and technology

dramatically over this time period. Even in the 8 years since the last edition,
there has been a major shift to a more aggressive surgical approach for the
treatment of some common fractures previously managed by conservative
methods. This change has been driven as much by health economics and
patient demand, as by improved long-term outcomes, and we continue to
describe both radical and conservative solutions, where appropriate
The changes to the content and format also reflect changes in the potential
readership since the book, which was once aimed primarily -at the medical
student, ii now used by a broad range of health professionals, including basic
surgical trainees. We have not attempted to produce a volume aimed at the
rp"iiulirt orthopaedic surgeon, although much of the content goes beyond the
needs of the student or basic practitioner. Some of this additional information
was previously presented in i smaller size of the print, but 1n t!9 edition all text
has been changed to a standard font format. It is hoped that this"will allow the

alternative. We have added one new chapter to address the important topic of

now recognised as a major health problem with a worldwide impact, resulting
from the increased longevity in ageing populations.
Many of the illustrations have been updated to reflect the current treatment
trends, but the anatomical classification of injuries has been retained and
enhanced. We feel that this provides a more readily understandable basis for
the general reader in preference to the more complex treatment-based classifications
used by the Specialist surgeon. The future direction of fracture treatment
is difficult to predict, but some of the newer techniques resulting from
developments in cell biology and molecular genetics that allow local
stimulation of bone healing has been included.
